Patient Particulars - Age- 50 year, height 5.2,,weight - 45,sex -female

History of Illness- 6 month pehle mujhe suddenly seene me dard hua night me.mene us din bahut jyada tention le li hi shayad es wajah se mujhe chest pain hone laga.or hanth pero me swelling aa hai.or bahut jayada ghabrahat ho rahi thi mujhe bahut passena bhi aa raha the.mujhe vomiting ,loose motion bhi bahut huye. Almost 7-8 vimitting hui 1 din me,fir mene pass me local docter ki dhikhaya to mujhe waha admit kar liya gaya. Me waha 8 din admit rahi or aaram bhi mila vomiting band ho loose motion bhi band ho gaye.fir me discharge ho gai.

30 din baad mujhe fir suddenly chest pain hone laga,saanse bhi bahut jyada bad gai thi or ghabrahat bhi hone lagi fir mene People's hospital me check karaya to bataya ki aapka dil kamjor hai.weakness bhi bahut jyada hai.7-8 din me people's me admit rahi aur aaram lag gaya.

Physician's viewpoint: Appears to have had a myocardial infarction and suffers from severe left ventricular dysfuntion (as demonstrated on Echo) since then. Also relates to have had 2 episodes of what appears to be paroxysmal nocturnal dyspnoea.

Asked to continue preload, afterload reducing agents along with secondary prophylactics in the form of statins and anti-platelets.
