Shortness of breath x 4 monthsFever (on/off)
As per the patient, he was apparently alright 4 mnths back when he developed shortness of breath which was gradual in onset. Progresion on going upstairs, walking, running. Relieved on taking rest. Took treatment from a local hospital and got temporary relief. Came for further treatment.
Chronic bidi smoker x 20 yrs
Alcohol x 3 yrs
Rs examination-
Bilateral fine crepts
Bilateral ronchi present
2d echo- severe mitral stenosis, mild AR, moderate PAH
