62 yrs man residing at Raisen near Bhopal.
he presented at pcms medicine male ward with following complaints:
pain in the epigastric and paraumbilical region since past 2 mnths.
PAIN WAS LOCALISED CONTINOUS MILD D GRADUAL ON ONSET PAIN WAS NoT AGGRAVATED BY CHANGE OF POSTURE OR
PASSING STOOLS .
personal h/o:chronic alcohol intake since 30 yrs .
o/e : p/a:non tender palpable mass was felt at paraumbilical region
non tender hepatomegaly present
advised :usg whole abdomen
imperssion of usg reports:1.liver enlarged 17.2 cm
2.cbd normal
3.pancreas and d gb normal
4.kidney lt normal rt 4mm renal calculi found
Presence of lobulated hyperechoic nodal mass ,mutiple matted lymph nodes in the periportal +peripancretic area 4.3 x 5.3
he also presented in opd
Other Clinical findings :head nodding
termors
advised : for fnac and cect abdomen
Due to unaffordability he could not get himself investigated further. Was prescribed oral analgesics (Paracetamol and Tramadol combination) and sent home.
