Pain and Swelling in Joints

History taken on 27th January, 2013

Chief Complaints - Swelling, pain and itching in all the joints of the body including the wrist and the knee joint as well as the tempero mandibular joint, unable to walk, eat or use hands due to excruciating pain - gradually progressing from the age of 16 - decreased in intensity on ayurvedic medications - but recurs on leaving the medication.

Past History - Ulceration on the scalp at the age of 10, relieved on Ayurvedic medications. Dyspepsia was frequent and the patient also complains of frequent stomach aches as well. Bladder, bowel, appetite normal though deviation in sleep pattern due to the pain.

After looking through the symptoms and reports, it seems that the most probable diagnosis is that of Rheumatoid Arthritis, which also seems to be there for 7 years ( since 16 years ). This diagnosis is also mentioned in the 2008 prescription.

Suggestions for the local doctor

X ray hands bilateral AP view

LFT, RFT and CBC with PS for comments

Tab Wysolone 10 mg once daily for 2 weeks

Tablet Naproxen 250 mg SOS for severe pain

Tablet Methotrexate 7.5 mg once weekly for 3 months and review


Follow up on 17th February, 2013 -





The patient is relieved of most symptoms, although the pain in the left leg still persists which makes him unable to work or perform any action. Dyspepsia still presents itself as well as frequent cough and cold, and deviation of normal sleep pattern.
