Patient particulars: Age 42 years; Height - 5' 4''; Weight - 60 kg; Sex - Female; BP - 120/80; Pulse -82;
Chief Complaint: Pain in lower abdomen and while micturition in the anal as well as the vaginal orifice and generalized fatigue and weakness
Past History
- When I was 14 years old, my periods started, would remain for 6 or 7 days and would be painful, irregular varying by 2 or 4 days. Also the colour was black.
- When I was 23 years old, I was married. Then I had one abortion and after four months, was pregnant with a baby girl, who was normally delivered. At 35 years, there was another abortion. I was then on a long term use of oral contraceptives but I have been off them for the past 3 years.
- Recurrent colds due to problem in tonsils.
- Have a problem in power in the eyes
- Suffers from Spondylitis and pain in shoulder as well as waist.
- Had about 4 or 5 separate episodes of urinary tract infection since 23 years. But this time there is intense pain during the infection. I had shown the doctor but still the pain and weakness, also in the lower abdomen persists.
Other symptoms: Appetite lost, dyspepsic, no interest in drinking water, bladder normal, bowel normal, sleep pattern normal
Follow Up on 16 Feb 2013.New Urine reports attached which shows the presence of an infection. Patient complains of weakness and pain in lower abdomen and also in urinary passages
